If you're without dental insurance, we offer a one-year, cost-effective plan for you and your family.

THE BENEFITS

- Two regular cleanings - You will be credited for a regular cleaning at your first two cleanings; then additional cleanings will be provided at 20% of the regular rate
- One examination
- Two fluoride treatments (for children up to age 14)
- One set of four bitewing films (plus two bitewing films per child)
- 20% savings for all dental treatments over $100 (excludes ortho care; no other cash savings will be honored)

THE PREMIUMS

$198 per person (a savings of $67 per adult and $101 per child)
$750 per family of four (savings of $330; additional family members at $10 per person)
Premiums are yearly and benefits are in effect for one year from the date that you sign for the plan
